- Search “Google Form” in google and enter to “Google Forms: Sign-in”.
- Sign in to your google user account.
- On the main page of google form, several ready-made templates will appear that you can use to create a pre-made form or create one from scratch(Blank form). In addition, additional templates are visible by clicking on “Template Gallery”.
In this guide we will use a blank form”. Click on “blank form”.
After you have opened the “blank form” you will see the title “Untitled form” by clicking on it with the mouse you can change the name of the form. Below it will appear “form description” to give a description to the form.
Before you edit the form, you must choose a name for the file on the upper left.
By clicking on any text in the form, you can design it with the help of the ruler(left to right): bold text, italics, underline, add a link, and delete all the formatting of the text to default.
On the right side of the form you will see a bar with several options.
The Options:
A. Add new Question.
B. Inserting Questions from another form from your google drive.
C. Add new Title and Description.
D. Inserting image from you google drive or from your computer.
E. Inserting video from youtube.
F. Cutting a form into several separate sections(example image below).
If you choose to split the form into several parts, you need to decide in each section what will happen after the end of the section. You should pay attention to the marker to continue to the next part as shown in the picture below.
Add new Question:
When you click on the plus icon in the right bar of the screen, a new window will open for you to add a question.
Write down the title of the question and you can insert a picture by clicking on the picture icon next to the title.
You can choose the style of the answers by clicking on “Multiple choice”.
To choose a title for the answer, move the mouse cursor over the word “option” and write down the title of the answer. To add another answer click on “add option”. below him.
At the bottom of the question you will see four icons(left to right): Duplicate question, remove the question, mark the question whether it is mandatory to answer it or not, more options.
If you click on the icon of the 3 points of the additional options, you can choose whether to add a question and a description for the question, you can add certain conditions when choosing a certain answer. For example, as soon as you click on one of the answers, it will direct the person filling the form to another section. And in addition a button(Shuffle option order) to change the order of the answers.
Responses Tab:
By clicking on the “Responses” tab, you can see the details of those who filled out the form. In addition, the details of the person who filled out the form can be linked to the excel form by clicking on “Link to sheets”.
When you click on “link to sheets” a pop-up window will open where you can choose to create a new excel file or insert the details of the person who filled out the form into an existing excel file.
You can also allow the person who filled out the form to respond to the form at the end of filling out the form by clicking on the “Accepting responses” button.
Settings Tab:
In the settings tab, you can define various settings for the form such as: whether to turn the form into a quiz with points and in addition to set how many points for each question, how to collect the details of those who filled out the form, whether to collect the email of those who filled out the form or not, the possibility of displaying The form is like a progress bar, etc.
Additional options of the form can be defined by the icons at the top of the form(left to right):
Puzzle icon – adding various plugins for free or for a fee to expand the possibilities of the form.
Color tray icon – general design of the form such as text colors, font size, background color, etc.
Eye icon – viewing the form in client view (not editable).
Back icon – deleting changes in the form.
Forward icon – returning changes to the form.
Send button – sending the form to a certain email address or addresses or receiving a direct link to the form or creating an iframe to embed the form on websites.
3 dots icon – additional possibilities of the form such as: duplicating the form, deleting the form, creating a direct link to the form, printing the form, adding additional users with a Google account to edit the form and unlocking various permissions, editing the code of the form, adding a new plugin and viewing shortcuts on the form’s keyboard.
